How health is derived

Rules
Healthy: heartbeat in the last 5 minutes and no queue/DLQ failure.
Stale: heartbeat between 5 and 15 minutes old.
Offline: heartbeat older than 15 minutes.
Failed: disabled instance, queue error, or DLQ messages present.
